Voluntary Participation & Assumption of Internet Risk
I, the undersigned participant (or parent/legal guardian of a minor under 18 years of age mentioned below), voluntary choose to enroll in and attend classes, workshops, community gatherings, somatic movement, breathwork, sound therapy, nature-enrichment activities and private sessions offered by MaMaitri LLC and Alexandra Loupatatzi Mallia. I recognize the movement, somatic movement, nervous system regulation involve physical exertion, stretching, cardiovuscular engagement, and emotional release. I knowingly and voluntarily accept full responsibility for all inherent risks of personal injury, physical discomfort, illness, or property damage that may occur during or after participation.
Educational Scope & Non-Medical Advice
I acknowledge that all offerings, instructions, and membership provided by MaMaitri LLC are strictly educational, complementary, and wellness-focused. Services do not provide medical diagnosis, physical therapy, chiropractic treatment, psychotherapy, psychological diagnosis, or licensed psychiatric care.
I confirm that I am in appropriate physical and psychological condition to engage in these practices safely. If I am pregnant, newly postpartum, or managing acute or chronic health condition, I certify that I have obtained medical clearance from a licensed physician or midwife prior to attending.
Release of Liability & Covenant Not to Sue
To the fullest extent permitted by Florida law, I knowingly release, waive, discharge, and hold harmless MaMaitri LLC, its founder Alexandra Loupatatzi Mallia, teachers, stuff, contractors, and the property owner of 45 Wilbur Drive, Unit 103, St. Augustine, Florida 32086, from any claims, demands, or causes of action, for bodily injury, emotional distress, illness, death, or property loss arising out of my presence on studio premises, attendance in outdoor nature sessions, or engagement in virtual platforms, whether resulting from ordinary negligence or inherent activity risks.
Premises & Personal Belonging
I assume full responsibility for navigating the physical studio space, parking areas, entryways, and restrooms. I understand that MaMaitri LLC is not responsible for lost, stolen, or damaged personal belonings brought into the facility or left in parled vehicle.
Refreshment & Allergen Notice
I understand that complimentary herbal teas, botanical infusions, filtered water, cottage-prepared refreshments, and pre-packaged snacks, privided on site are consumed at my own discretion. I accept sole responsibilty for confirming ingredients against my personal food allergies and dietary sensitivities prior to consumption.
Minor Participation Rider (Florida Statute 744.301)
Required for any participation of minor under the age of 18: if signing on behalf of a minor child, I certify that I am the biological parent or legal court-ordered guardian with lawful authority to execute this agreement.
Under Florida law, I grant permission for the minor of participation in studio classes, youth circles, and nature enrichment programs, and I explicitly release MaMaitri LLC from inherent risks associated with youth movement, process art, and group activities.
Participant/Guardian Signature & Acknowledgment
I have read and fully understand this Liability Waiver and Wellness Agreement. I sign it freely, voluntarily, and with full knowledge that I am agreeing to binding release of claims.
